The Royal Suit is the third dress to take on a skull as its bow, following the traditional Hollow Yves look. The pinafore features a spade picture as well as astronomical signs of Venus, and the astrological sign for Leo (a descending node). The bow is similar to her normal costume, however, the skull is replaced by a twisted heart-shaped skull. The gloves worn by Alice mimic castle towers and the dress has re-occurring prints of hearts and checker patterns. It is based around the Queen of Hearts and is only seen in Queensland. The suit has the effect of having more visibility in shrink sense. The bow is replaced with a pink bow that is parted in the center with a doll's head. The pockets have embroidered astronomical signs of Mars and Ceres (Mars represents Alice's battle, while Ceres represents both Alice's old house where this all began and her current miserable home which is the source of her trouble). She wears a red and black tartan skirt with decorative stitching along the border over a baby-pink, ruffled petticoat. The left sleeve has a tear with stuffing poking out of it. A vertical striped bodice with over-sized buttons can be seen, possibly to make some sort of plushie effect, as well as blue and white striped gloves. It reflects the color and vibrancy of childhood, particularly hers during the days of playtime with toys. She/He wears an Omega pendant on a silver chain around her neck, and the pockets of the apron have the astronomical signs of Eris and Jupiter sewn on them. She was given knee-high black boots, and a new pair of black and white striped stockings. There are blood stains on it from previous battles, and the bow at the back has a small skull in the center. It is knee-long with short-puffy sleeves, worn with a white pinafore over the top. This color was popularized by Disney's 1951 animated film. Like with most versions of Alice in the media, her dress is blue.

In the orphan's return, selecting this dress to wear has a effect of Alice constantly regaining health while using shrink sense.
